It is a horror game drawing inspiration from the lead designer Matt Gilgenbach's personal struggles with obsessive–compulsive disorder (OCD) and depression; in an interview, he stated that he was "trying to create that feeling [of bleakness and hopelessness] in Neverending Nightmares".
